Personal check or money order must be signed, dated and made payable to U.S. Department of State. Remittance must be payable in U.S. dollars through a U.S. Bank. No credit cards or cash accepted. Telephone or facsimile requests are not accepted. A signed and notarized written request must be submitted along with a copy of the requester's valid photo identification.
